Referral process

What happens from referral to the next step.

Four clear phases show what school sends, how suitability is considered, what a placement involves and how progress is reviewed.

01

Refer and discuss

Send the starting picture.

A school professional completes the short online form with the pupil’s key stage, attendance context, school connection, main barriers and support already in place.

pathways considers likely fit

A member of the team discusses the intended outcome and whether part-time support could add something useful alongside school.

02

Visit and agree

See the place before deciding.

If Pathways may be suitable, the pupil, parent or carer and a school representative can visit together while the setting is quiet.

One shared placement plan

  • Starting point and barriers
  • Pathways and school days
  • Attendance and engagement targets
  • Communication and review points
  • Transport, funding and practical arrangements
03

Attend and review

Keep school and Pathways working from the same plan.

The pupil attends Pathways for two or three agreed days. English, maths, structured learning, targeted support and personal-project work sit alongside their continued time at school.

Information moves both ways

Pathways shares attendance each day, alongside engagement, welfare updates and progress against agreed targets. School information shows how the pupil is managing across the rest of the week.

04

Plan what follows

Recognise progress and agree the next step.

A final showcase can celebrate work and reconnect important adults around something positive. Exit planning then focuses on stronger, sustainable participation with the pupil’s main education provider.

Progress is not pathways attendance alone

Reviews consider school attendance, overall attendance, punctuality, participation, confidence and progress against the pupil’s individual targets.
