What the initial form sends.
The form sends the referrer’s professional contact details and a short, non-identifying attendance picture to the pathways team.
- Do not include the pupil’s name, date of birth, address, medical records or directly identifying details.
- Detailed pupil information is requested only if the referral appears suitable and everyone wishes to proceed.
- The form is for routine referral enquiries, not immediate safeguarding concerns.
- Request the current privacy information if you need details about handling, retention or individual rights.
For a placement
One pack for the responsibilities that vary by pupil.
The individual agreement should confirm the details below before attendance begins.
Follow your organisation’s safeguarding procedure or contact the appropriate emergency service. Do not use the website referral form to report an urgent safeguarding concern.
